We want to ensure new students follow the appropriate health requirements as they prepare for the 21-22 school year. The following forms apply to students in every division, from infants to 8th grade.

Review the forms listed below in advance to schedule your doctor appointments and arrange for the pertinent paperwork. Make sure all forms include the appropriate signatures and completed fields.

Submit all forms to by the first day of school. Your child will not be admitted to school until the Health Form and the Immunization Records are filed in the nurse's office. Please do not hesitate to contact us with any questions.

Neely Besetsny, R.N., B.S.N.
Larissa Broussard, R.N., B.S.N.
Carrie Frain, R.N., M.S.N.

Parents and a physician must sign this form by the first day of school. Bring this health form to the physician's office during the examination to complete all fields. Scan or take a picture of the signed document and email to the nurses. We recommend for parents to book a physical during the summer and provide the annual form by the deadline.

Provide an updated copy attached to the Health Form by August 1, 2021. The nurses will review all health records and report to the Texas State Public Health Department to ensure each student is compliant with state requirements. All students, new or returning, must provide an updated immunization record and physician health form every year.

Middle School students trying out for a school athletic team must provide a Sports Physical Evaluation and Authorization Form. Complete the form and email to the Athletic Director, along with a copy of the Health Form signed by a physician, by August 1, 2021.

View and print the letter from the nurses that contains all the information in a single page. Contact Nurse Neely, Nurse Carrie or Nurse Larissa for questions or comments.
