STUDENT HEALTH RECORDS
We want to ensure new students follow the appropriate health requirements as they prepare for the 20-21 school year. The following forms apply to students in every division, from infants to 8th grade.
Review the forms listed below in advance to schedule your doctor appointments and arrange for the pertinent paperwork. Make sure all forms include the appropriate signatures and completed fields.
Submit all forms to nurse@hses.org by the first day of school, August 19, 2020. Your child will not be admitted to school until the Health Form and the Immunization Records are filed in the nurse's office. Please do not hesitate to contact us with any questions.
SCHOOL APP
Stay connected through your mobile device and access all information in one spot! Download the HSES app by scanning the QR code, searching the app store for android or apple, or click this link.
Once downloaded to your device, open the app and follow the instructions for set up by selecting the groups of your interest. Make sure you select the “Schoolwide” option and enable notifications to receive alerts and important information.

The family portal provides access to student records and allows parents to manage their personal information, including applications and enrollment.

DENNIS UNIFORM
HSES requires a daily uniform for students in the 2s classroom through 8th grade. Parents can purchase new uniforms through Dennis Uniform or used uniforms through the PTO. The Dennis Uniform website includes specific guidelines per grade level and gender. The guides also listed below detail the required and optional dress codes. PTO USED UNIFORM
Parents interested in purchasing used uniforms can complete an order form for the PTO team to prepare and keep track of orders. Since only a limited supply is available, this method will help to keep a record of inventory and purchases. Contact the PTO parent volunteer for questions.

P.E. UNIFORM — Middle School Only
The INSTRUCTIONS highlight the required athletic gear but no longer allow for orders since the deadline has passed. Contact the Director of Athletics for P.E. uniform details.
SCHOOL SUPPLIES
Online ordering is now available for school supplies. You can save up to 40% by pre-ordering Primary 3 through 8th grade supply packs. Supplies will be delivered directly to the classroom for students to retrieve on the first day of school.
